Please begin the registration process by downloading the following forms, instructions, and commodity code directory.

• Instructions for completing form DT1025
• Veteran Owned Business Request for Certification Form (if applicable)

Commodity code directory:

• Index-sorted by commodity description (551 KB)
• Numeric-sorted by commodity code (579 KB)

(Note: this directory is a total of 110 pages in length so, if necessary, please only print the pages that apply to your business).

Complete both sides of the forms and send to the address indicated on the form.

Please note: File sizes for files under 250 KB are not noted on this page.
