Wisconsin law requires applicants for a duplicate license to have legal presence in the United States.

If you are not a U.S. citizen or do not have a valid social security number on file with the Department of Transportation, Division of Motor Vehicles (DMV) verified through the Social Security Administration (the DMV automatically verifies Social Security numbers), you must appear in person and present acceptable proof of U.S. citizenship, legal permanent resident status, legal conditional resident status or legal presence in the U.S. to obtain a duplicate license.

If you are a Wisconsin resident and a U.S. citizen with a valid Social Security number on file verified through the Social Security Administration, you are temporarily out-of-state and your license has been lost or stolen, you can obtain a duplicate license through the mail by completing and following the instruction on the Wisconsin Driver License (DL)/Identification Card (ID) Application (form MV3001) PDF.

You can not obtain a replacement ID card through the mail. ID cards may only be issued at a Wisconsin DMV Service Center.

Make check or money order for the fee payable to "Registration Fee Trust" along with the temporary out-of-state mailing address to mail back your duplicate license. Mail the application and fee:

Expedited delivery service provides faster delivery service of the license to the address you provide; however, it does not expedite the Division of Motor Vehicle's processing time as licenses can be mailed no sooner than the day after we process your request.

If you wish to use expedited delivery service, send a self-addressed, postage-prepaid, overnight envelope in which we can send your duplicate license to you, the required fee and completed form MV3001 PDF to:
