If your license or identification card was recently lost or stolen, you can apply for a duplicate either online or at any Division of Motor Vehicles (DMV) service center (excluding renewal-only offices). Out of state addresses or special mailing addresses are not allowed via the online application.

If your license was lost or stolen, and you are temporarily out of state, you can replace the license through the mail using your temporary out of state address.

Identity theft

If you have reason to believe your identity has been stolen, contact your local law enforcement agency immediately. 

The Wisconsin Office of Privacy Protection is a resource center for individuals, businesses, law enforcement and government on issues of privacy protection and identity theft.

You can also ask the DMV to place a notation on your driver record so that DMV and law enforcement will require additional identification documents when you conduct business with them. The statement will be “verify identification documents,” and it will require anyone (including you) attempting to use your identity to provide the following items:

  1. A certified birth certificate and
  2. Two forms of identification listing your name with either your photograph or signature.
