Lost renewal notice - driver license, ID card

Driver License and identification card renewal notices are mailed to the last known address 45-60 days prior to the expiration date. If you have lost your notice or if you did not receive one, you may apply for renewal at any DMV service center. Completely fill out form MV3001 PDF (561 KB) - Wisconsin Driver License Application or MV3004 PDF (405 KB) - Wisconsin Identification Card (ID) Application, and use your current driver license or ID card for identification.

Drivers who fail to renew their license by their birthday will be assessed a late fee in addition to the usual driver license fee.

A regular driver license may be renewed up to 1 year prior to the expiration date. A probationary license can be renewed 90 days prior to the expiration date.
